How to be Referred
Most referrals to Saint Francis Hospice come via healthcare professionals such as GPs or district nurses and we encourage you to contact your own healthcare advisor.
We can take a referral direct from a patient or, with their permission, from family or friends, but the hospice would need to speak to your healthcare advisor and so this approach can take longer.
Referrals can be made for day care, our Hospice at Home service and inpatient care, although we are unable to offer respite care due to the demand on our services for acute care. Every referral is assessed by our team and places allocated on a basis of need. Patients who receive services are continually assessed so that we can understand whether their needs have changed and how best we can respond.
We make every effort to ensure that patients receive individual care that is right for them. We will discuss your care with you on the phone before confirming it in writing so that you have all the details you need. We also liaise with your GP, district nurse or any other healthcare professional involved.
