Community Palliative Care Team (CPCT) and Triage
With 13 clinical nurse specialists, the CPCT offers advice and support to patients on a 24/7 basis. Out of hours an emergency ‘on call’ service is provided.
The CPCT nurses are skilled and experienced in controlling symptoms and caring for those with life limiting illnesses whilst they remain in their own homes. They work alongside district nurses, GPs, social services and others involved in your care and provide additional services to those already being offered through your primary health care team.
Upon referral your need will be assessed by our triage team who carry out, on average, 450 new assessments every year.
