We care for 500 in patients and make more than 5,000 home visits a year.

Triage
The triage service is provided during the hours of 9.30am to 5.00pm each weekday. It is often the first point of contact from hospice to patient.
On receipt of a referral, the triage nurse specialist will ring to explore what problems there are, offer advice and support and then refer to the most appropriate hospice service. Many patients who do not need regular visits from the community palliative care team but may have problems or questions related to their illness find the support of our advice but not visiting service more than meets their needs.
The triage team also provide a specialist support helpline for professionals or any member of the primary care team caring for patients with life-limiting illnesses in the community.
