Hospice at Home Nursing Team
Many people who are suffering from cancer or another advanced illness want, above all, to remain in their own homes and to die at home.
They may, however, have worries about how they and their carers will cope should their condition deteriorate or a crisis arise.
Our hospice at home service works with the care you are already receiving, such as district nurses, to provide you with specialist care for periods of between 2 and 24 hours, dependent upon assessment. Our team of staff nurses and health care assistants will offer support to help prevent hospital admission and to manage a crisis.
A referral may come via your own nurse or doctor but you can contact the Hospice at Home Co-ordinator, Tracie Brennan, on 01708 758603 from Monday to Friday 9.00am to 5.00pm. Outside of these hours contact can be made via your district nurse. Alternatively, you can email Tracie.
