Care At Home
For many people, staying in their own home is a priority. With the right support, care and attention from medical staff, this is often a real possibility and can give enormous comfort to the patient as well as allow family and friends to be close by.
Our Outreach/Community Services
Our Hospice at Home service supports both patient and carer seven days a week, 24 hours a day. We assess your requirements for care and allocate our service on a basis of clinical need; this assessment is ongoing and reviewed daily. Care is provided in ‘blocks’ of between two and four hours; these can run sequentially so that patients who need intensive support can be provided with 24 hour care.
All patients can benefit from the range of therapies we offer, including occupational therapy, physiotherapy, complimentary therapy, chaplaincy and bereavement support.
The Hospice at Home team works with your GP, district nurse and any other healthcare professionals involved so that you receive the care you need at the time you need it.
Hospice at Home Nursing Team
Many people who are suffering from cancer or another advanced illness want, above all, to remain in their own homes and to die at home. They may, however, have worries about how they and their carers will cope should their condition deteriorate or a crisis arise.
Our hospice at home service works with the care you are already receiving, such as district nurses, to provide you with specialist care for periods of between 2 and 24 hours, dependent upon assessment. Our team of staff nurses and health care assistants will offer support to help prevent hospital admission and to manage a crisis.
A referral may come via your own nurse or doctor but you can contact the Hospice at Home Co-ordinator, Tracie Brennan, on 01708 758603 from Monday to Friday 9.00am to 5.00pm. Outside of these hours contact can be made via your district nurse. Alternatively, you can email Tracie: mail@sfh.org.uk
Community Palliative Care Team (CPCT) and Triage
With 13 clinical nurse specialists, the CPCT offers advice and support to patients on a 24/7 basis. Out of hours an emergency ‘on call’ service is provided.
The CPCT nurses are skilled and experienced in controlling symptoms and caring for those with life limiting illnesses whilst they remain in their own homes. They work alongside district nurses, GPs, social services and others involved in your care and provide additional services to those already being offered through your primary health care team.
