The Hospice provides specialist advice, support and facilitation of care for those persons with complex symptom management due life limiting illness and support to carers of the individual within the home setting or at a nurse led clinic environment.

An organisational opportunity has arisen for a dynamic, enthusiastic, self-motivated, experienced palliative care nurse to join our developing Referrals Hub. This post will provide an opportunity and experience to develop further skills and knowledge in being the first point of contact in referrals to the organisation and work alongside our Referrals & Admissions Coordinator and Referrals Lead.

We work within the boundaries of Barking & Dagenham, Havering, and Brentwood and out of hour’s advisory support to Redbridge, which operates as part of the wider multidisciplinary team, working collaboratively with other internal and external health care professionals.

We are looking for a candidate who has excellent communication, decision-making and prioritisation skills, and understands the needs of those receiving palliative care /end of life care with a working knowledge of evidence based practice and the ability to develop a good knowledge base of current initiatives and developments in palliative care. Any community working experience would be taken into account.

Relevant experience of working in a more senior role is essential with appropriate supporting qualifications and as well as any evidence of appropriate leadership qualities.

A structured on going supportive development and mentorship programme would be offered, which would involve successful completion of agreed palliative care competencies.
