An exciting opportunity has arisen for an experienced clinical practitioner to join Saint Francis Hospice in a newly revised role to support the Single Point of Access (SPA) & Referrals Hub Service -’the front door’ for all referrals to the hospice- in ensuring skilled, efficient and effective triaging.

Do you have excellent clinical skills and a passion for delivering Person Centred Palliative Care?

Saint Francis Hospice is a dynamic, forward-thinking organization delivering a full range of specialist palliative care services for all individuals with life limiting illness. We achieved ‘Outstanding’ in our CQC inspection in August 2024 maintaining what we achieved in 2016. We have recently adopted an exciting 5-year strategy to reconfigure our services to meet the changing needs of our local community and ensure we are reaching all individuals that require palliative care expertise.

The hospice supports the extensive local area including Havering, Barking & Dagenham, Redbridge, Brentwood and West Essex and has a robust governance structure in place. We provide onsite clinical services within a modern, personal 18-bedded Ward, as well as Specialist Community Crisis & Support Service (SCCS), Hospice at Home Service and Specialist Therapies Outpatient Services provided at Pemberton Place. Alongside these clinical services, we have the Pepperell Education Centre that has an excellent reputation for delivering high quality education programs and study days to support professional learning.

The post holder will be responsible for individual holistic clinical assessment of individuals referred and will promote clinical leadership, demonstrate high standard of skills, whilst ensuring safe and effective practice over a flexible 7-day rota. They will enhance the service user experience and contribute to effective leadership/collaborative team working and contribute to the delivery of the organization’s objectives and new strategy.

The clinical specialist role will work closely with the pSPA & Referrals Hub Manager, Referrals Hub Clinical Nurse Specialist, Referrals Assistant and Referrals & Admissions Coordinator.

The Referrals Hub Service will work pro-actively across all service areas within Saint Francis Hospice to ensure that all plans and arrangements following assessment are in place for a person’s acceptance and admission to appropriate hospice services.

If you are a passionate, highly motivated and dynamic individual with specialist clinical experience in palliative care as well as being a team player with strong interpersonal and communication skills, we would love to hear from you.

In return we will offer supervision, reflective practice, staff support, professional development opportunities and a supportive working environment. Transfer of NHS superannuation scheme or a private pension scheme is also available.

Please note that a DBS (Disclosure & Barring Service) criminal record check will be undertaken as part of our pre-employment screening for the successful candidate.

If you are excited by this opportunity and would like to discuss this opportunity or arrange an informal visit, we would be delighted to hear from you. Please contact Lesley Burrows Head of Community Services or pSPA & Referrals Hub Manager April Burrell on 01708 753319
