Enhanced Hospice at Home
Our Enhanced Hospice at Home service enables residents to remain in the comfort of their familiar surroundings while receiving specialist support.
What You Can Expect
- Care provided 7 days a week, 9am - 9pm
- Daily prioritisation based on clinical need
Collaborative working with your nursing home team
Specialist support with symptom management and end-of-life care planning
This service ensures residents receive the right care at the right time, delivered with compassion and expertise.

Hospice Community Team
Our Community Team provides specialist palliative care advice and guidance for residents, loved ones and healthcare professionals.
24-Hour Specialist Advice Line
We offer round-the-clock access to expert clinical support.
- Available 24/7 for urgent palliative care advice
- Voicemail service for non-urgent messages outside office hours
- Clinical Nurse Specialists on call remotely between 5pm–9am
