You can stay in your own home with the people who matter to you and a team of compassionate professionals to help at any time.
The Enhanced Hospice at Home team can support you in the comfort of your own home or a care home. You will receive the same standard of personalised palliative care that you would receive in the hospice Ward. The Enhanced Hospice at Home team offers care for seven days a week.
The district nurse will be your main contact, or ‘key worker’ and the Enhanced Hospice at Home team will work alongside them and any other healthcare professionals involved in your care. The Enhanced Hospice at Home team do not replace any services you are already receiving.
The Enhanced Hospice at Home team will review your care at each visit and will re-assess the support you are receiving at six weekly intervals. Ongoing support will be provided according to specialist palliative care need.
The Enhanced Hospice at Home team works in a holistic way and will liaise with your district nurse, any other healthcare professionals involved in your care and other members of the multidisciplinary team at the hospice to help ensure your needs are met. You will receive the same standard of personalised palliative care you'd receive in the hospice.
Your Enhanced Hospice at Home team works alongside your current health and social care professionals. You'll be able to have all your medical, emotional and spiritual needs taken care of.
