Advance Care Planning (ACP) is the process of discussing, planning ahead and documenting in anticipation of deterioration in a person’s condition.

Who is this for?
This study day is for all healthcare providers who work with people with life-limiting illness and end of life care issues. It is suitable for both registered and non-registered staff from any care setting.
